Use this form to apply for a cash grant of up to $150,000.

This application must be completed and submitted in one session. You will not be able to save your progress and return to it later. Before you begin, please ensure you have all required information and supporting documents ready to upload.

Required documents

To complete this form, you will need to submit the documents listed below. Attach only the required documents. Documents submitted separately from the application will not be considered.

  • Project need and design
    • A one-page summary that explains how your project aligns with your organization's mission.
    • Proof of non-profit registry and status.
    • Combine all documents into a single file, highlighting relevant sections clearly.  
    • File: 1 MB limit. Allowed types: pdf, doc, docx, xls, xlsx.
  • Project proposed budget
    • Download and complete the template. (Right click to save the template if your browser’s security settings prevent download by clicking the link.) Include itemized expenses and all funding sources, including cash and in-kind support. Identify which funding is confirmed.
    • File: 1 MB limit. Allowed type: xlsx.
