Property owners are invited to submit their own financial statements in response to this request by filling out the certification below and attaching their own documentation.  Email confirmation of submission is provided.

In the event that you are responding with information on more than one property, please provide separate submissions for each property roll number.

The collection of this information is authorized by sections 16(1) and 16(2) of The Municipal Assessment Act which, upon request, require you to provide the City with information or documentation that may relate to the assessment of a property.  Failure to respond may result in penalties under The Municipal Assessment Act. The personal information you provide is protected from unauthorized use and disclosure under The Freedom of Information and Protection of Privacy Act, Part 3, and it will not be used or disclosed for any other purpose, except as authorized by law.
